Please take both copies of the PO to the store when you go.
This is not a blanket purchase order; you have 30 days to utilize a pickup. Please note the effective date on the PO. You must use by this date, or the PO will be cancelled.
This Purchase Order is for consumable supplies only!
- IT WILL NOT COVER EQUIPMENT PURCHASES, OR ANY CHEMICALS CONTAINING HAZARDOUS SUBSTANCES. (this includes Appliances, TVs, etc)
- ***NOT VALID FOR PURCHASE OF GIFTCARDS***
Please return your signed receipt and copy of signed PO to Accounting within 3 days of purchase
The signed receipt must show purchase the order number, prices, and discounts.
